Walmart adds refrigerated prescriptions to same-day pharmacy delivery

Walmart is offering more medications for same-day delivery.

Walmart is expanding the range of prescription medications it will deliver alongside groceries and other items in a single online order.

Same-day pharmacy delivery from Walmart, which was initially launched to provide delivery of prescription medications in as little as 30 minutes in October 2024 and offered nationwide in January 2025, now includes refrigerated and reconstituted medications — such as insulin, GLP-1s and pediatric amoxicillin — and is available across the U.S.

With this move, Walmart says it becomes the first retailer to deliver refrigerated and reconstituted prescriptions alongside groceries and everyday essentials in a single online order.

"We continue to unlock new levels of convenience while also broadening access to critical medications," said Kevin Host, senior VP of pharmacy at Walmart. "Adding refrigerated prescriptions to our pharmacy delivery capabilities is the result of listening to our customers, identifying where we can create even better, seamless experiences and keeping the communities we serve at the heart of what we do."

According to Walmart, it has fulfilled more than 4 million pharmacy delivery orders, the fastest occurring in nine minutes. The discount giant says refrigerated medications represent more than 30% of its pharmacy sales.

Once Walmart fills a prescription, customers receive a notification on their pharmacy account. From there, they can choose same-day scheduled, on-demand or express delivery. Insurance is applied automatically, just as it would at the store, and customers must sign for their delivery via electronic signature. 

Other features of Walmart’s same-day prescription delivery offering include real-time order tracking in the Walmart app and insulated packaging with light-blocking materials for medications.

The same-day pharmacy delivery network is compliant with federal HIPAA laws and connected to Walmart’s AI-based Spark delivery platform. Spark is now using new geospatial technology to define delivery catchments (areas) with greater precision. 

Walmart customizes these catchments for each store using customer insights and external data, which it says outperforms traditional metrics such as ZIP code or distance from the store. 

Members of the Walmart+ paid loyalty program will receive free pharmacy delivery as a membership benefit. Non-members will pay a fee based on delivery preference. 

In May 2025, Walmart opened a 102,000-sq.-ft. facility in Frederick, Md. designed to fulfill up to 100,000 prescriptions a day, supporting more than 700 stores across 16 states and Washington, D.C.

This facility uses advanced pharmacy automation with dynamic weighting systems, robotic carriers and a conveyance route designed to streamline every step of prescription fulfillment, from pill counting and labeling to capping and sorting. It employs pharmacists, pharmacy technicians and patient safety associates to provide expert oversight.

With this launch, Walmart continues to expand its central fill operations to meet growing demand for pharmacy services, as well as pharmacist-provided clinical services including immunizations and testing and treatment for strep throat, flu and COVID.

Two more pharmacy central fill facilities are in development — one in Phoenix and another in Republic, Mo. — and are expected to open in 2026.
